At a hold point work cannot proceed until the inspector attends and signs off — it is mandatory. At a witness point the inspector is invited to attend, but work may continue if they cannot, provided notice was given. Both are defined in the inspection and test plan (ITP).
We log each item with a location, description, category and photo, then track it to closure and re-verify the fix on site. Category A items (blocking start-up) are cleared before commissioning; Category B items are closed to an agreed schedule.
